Cost:

• $250.00 for complete (upper & lower).
• This includes custom trays plus one Colgate Platinum Kit.
• Payment is due in full at the time treatment commences.
• If custom tray is lost or damaged there will be a $35 charge to make a new tray.
• If new impression is needed with new custom tray, there will be a $75 replacement charge.
• Additional whitening paste is available at $10.00 a syringe.
• Upper bleach tray only-$180.00.
• At home bleaching kit- bleach paste is put into trays and worn for 1½ - 2 hours per day for 2 weeks, or until desired whiteness is achieved.
